In our Hospice program, we provide individual attention to each patient and family, working toward providing as much relief from physical, emotional, and spiritual stress as is possible. Listed below are some highlights from our Hospice services:

Medical Resources
We are able to provide direct access to the medical equipment and medication that may be needed for care. This service helps to relieve much of the stress associated with trying to procure all the needed resources.

Providing Choices
We honor the wishes of the patient and the family and will work together to find ways to reduce emotional stress. Our Hospice team is available to work ly with families and loved ones to ensure the comfort and dignity of the patient. We respect that each family has its’ own ways of caring for one another and that each situation is unique.

Palliative Care
We believe that Palliative Care is a vital approach toward relief from symptoms and improving the quality of a patient’s experience. We are committed to education toward alleviating physical discomfort and stress in our patients. Support for the Caregiver
It is vital that the caregiver is supported, healthy and able to have a balanced schedule whenever possible. Our team offers supportive services in homemaking and Caregiver support. In-Patient Respite Care also offers flexibility to the caregiver for attending out-of-town events or family trips.

Volunteer Support
Our trained volunteers communicate with health professionals to help to make this experience as comfortable and peaceful as possible.

Pastoral & Spiritual Care
We assist each patient with connecting to spiritual care that is right for that person. We are here to be supportive of your choices.

Bereavement Counseling
Our Hospice program continues to provide support the family in bereavement counseling and educational outreach on bereavement issues. For up to a year after a loved one has passed away, families continue to receive our Journeys newsletter, which provides helpful articles about bereavement issues.

We help to reduce day-to-day stress by providing multiple levels of support to patients, caregivers, families, and loved ones. We help to educate you about the choices you have and the resources available to you. We assemble an interdisciplinary team based on your individual needs.

Since 1967, Pemi-Baker Community Health has been a provider of skilled, compassionate, progressive home-based health services to our community members. Many medical services that were once only available in a hospital or rehabilitation facility are now safely and effectively administered by our staff who are experts in home care.
