Pemi-Baker Home Health Program
Pemi-Baker Community Health is a Medicare certified organization and therefore provides intermittent, short term rehabilitative nursing and therapeutic care (physical therapy, occupational therapy, and speech therapy) as well as short term in-home support (Licensed Nursing Assistants).

Long term care is a Medicaid program called Choices for Independence for which we are a provider.  There are financial and medical requirements to qualify for this program.

The purpose of home care is to restore or to maintain a patient at their highest possible level of health, function and comfort. Home care is for all ages and needs, short duration or long, and is intermittent.

We welcome members of the community to our heated therapeutic pool for classes and design-assisted exercise programs. Our fitness training gym is open for use by the public by members or drop-ins with flexible hours and personal trainers by appointment.

  • We give personal attention to all patients and families and work as a team toward the goals of health, safety, and independence
  • We encourage patients to be active partners in their treatment and rehabilitation
  • We work with the patient’s physician to develop a comprehensive care plan
  • We support the family by providing help, reassurance and support as they cope with the challenging and rewarding tasks of caring for a loved one at home.

Since 1967, Pemi-Baker Home Health has been a provider of skilled, compassionate, progressive home-based health services to our community members. Many medical services that were once only available in a hospital or rehabilitation facility are now safely and effectively administered by our staff who are experts in home care.
